New Patient Checklist

Complete before your first visit

  • Complete your intake forms

You will receive a link after booking. Fill these out thoroughly — the more detail you provide, the more productive your visit will be.

  • Gather recent lab results (if any)

Any bloodwork from the past 6–12 months is helpful. Upload directly through the intake portal or bring a printed copy.

  • Write down all current medications

Include prescription medications, over-the-counter drugs, supplements, vitamins, and herbal products — with dosages if possible.

  • Note your symptom timeline

When did symptoms start? Have they changed over time? What makes them better or worse? Writing this down helps you communicate clearly.

  • Prepare your questions

Write down everything you want to ask. There are no silly questions. We encourage you to bring a list so nothing gets forgotten.

  • Confirm your tech setup

Test your camera and microphone before the visit. You will need a stable internet connection and a quiet, private space.

  • Have your pharmacy information ready

Name, address, and phone number of your preferred pharmacy — local or mail-order — in case prescriptions are needed.

  • Upload any prior specialist notes

If you have seen other specialists for related concerns, those records can be very helpful context for your endocrinologist.

Tip: Use your browser's print function (Ctrl+P / Cmd+P) to save this checklist as a PDF or print it out. Bring it to your visit or keep it on your phone for easy reference.

Common Questions

Pre-Visit FAQ

Answers to the most common questions patients have before their first visit.

Do I need a referral to schedule an appointment?

No, you do not need a referral. We welcome self-referred patients. If you suspect a hormonal or metabolic issue is affecting your health, that is a valid reason to schedule a consultation.

Do I need a diagnosis to be seen?

Absolutely not. Many of our patients come to us precisely because they have unexplained symptoms and no clear diagnosis. A thorough endocrine evaluation is designed to uncover conditions that may have been missed elsewhere.

What should I bring to my first visit?

Complete your intake forms before the visit. If you have recent lab results (within the past 6–12 months), upload them through the portal or have them ready to share. Bring a list of all current medications, supplements, and vitamins with dosages. Write down your symptom timeline and any questions you want to ask.

How long is the first visit?

Initial consultations are 60 minutes. We structure our schedule to give you the time you need without feeling rushed. Follow-up visits are 30 minutes.

What if I have never had labs done for my symptoms?

That is completely fine. During your first visit, your endocrinologist will take a detailed history and determine which labs are appropriate based on your symptoms and concerns. Lab orders will be sent electronically to a convenient location near you after the visit.

How does a telehealth visit work technically?

You will receive a secure visit link via email. Click the link at your appointment time from a computer, tablet, or smartphone. You will need a stable internet connection, a working camera and microphone, and a quiet, private space. We recommend testing your setup a few minutes early.

What if my symptoms are vague or hard to describe?

Vague symptoms are common with hormonal and metabolic conditions — fatigue, brain fog, mood changes, and weight changes can all be difficult to pin down. That is exactly what we are trained to evaluate. Write down what you are experiencing as best you can, and we will work through it together during the visit.

What if I am already on medications or supplements?

Bring a complete list of everything you are currently taking, including prescription medications, over-the-counter drugs, supplements, vitamins, and herbal products. Include dosages if possible. This information is essential for understanding your current health picture and planning any changes.

Can I bring someone with me to the visit?

Yes, you are welcome to have a family member or partner join you during your telehealth visit. Some patients find it helpful to have someone else listen and take notes, especially during the first visit when a lot of information is covered.

What happens after my first visit?

You will receive a written visit summary with recommendations sent securely to you. If labs are needed, orders will be sent electronically to a lab location near you. Prescriptions, if appropriate, will be sent to your preferred pharmacy. A follow-up appointment will be scheduled as needed to review results and finalize your care plan.
